Home visits across the Hills District growth corridor for older clients, Support at Home recipients, and anyone who finds getting to a clinic difficult, particularly in the early weeks after a hospital stay or surgery.

Who home visits suit

Built for older adults, post-op recovery, and family members coordinating care.

  • Older adults at home

    Patients who find clinic visits tiring or impractical. Falls prevention, strengthening, mobility, and ongoing maintenance in your own living room.

  • After surgery or hospital

    The first weeks after hip or knee replacement, spinal surgery, or any procedure where leaving the house is difficult. Surgeon protocols followed carefully.

  • Family coordinating care

    Adult children organising support for a parent. Happy to attend reviews, send brief updates after each session, and coordinate with GPs or care managers.

Support at Home

Government-funded in-home physiotherapy for older Australians

Support at Home replaced the Home Care Package program on 1 November 2025. Physiotherapy is funded under Clinical Supports with no patient contribution for eligible older adults. If you (or a parent) already have an approved Support at Home plan, we can work alongside your care manager to schedule visits.

  • Eligibility starts with a free assessment via My Aged Care
  • Physiotherapy sits under Clinical Supports
  • Visits can be weekly, fortnightly, or as agreed in your plan
  • Family members can attend appointments alongside the patient
